For Texas PAS agencies · Licensed under 26 TAC ch. 558

The QAPI program a Texas PAS agency can actually produce.

Indicators, incident and ANE reporting, the emergency preparedness checklist, training records, and performance improvement projects — every requirement of 26 TAC Chapter 558 wired to the record that proves it, and compiled into a survey-ready PDF in one click.

02Ask the Rulebook

Ask the Rulebook.

Every Chapter 558 and EVV question, answered with the actual citation — in seconds, not a consultant call.

Every citation is checked against our regulatory registry before it renders. When an answer can’t be grounded in the Sourcebook, PasQAPI says so rather than inventing a section number.

Ask a questionIncluded free at launch
PasQAPI · RulebookExample

Can my attendant transport a client in their own car?

Transportation isn’t one of the tasks §558.404(d) permits under a PAS category, so your licence doesn’t authorize it on its own.

If you allow it anyway, that’s an agency policy decision — your insurer and your own written policy govern it, not the licence.

Put the decision in the client’s individualized service plan under §558.404(f), so a surveyor reads a documented choice instead of an improvisation.

03It does the writing

It does the writing.

Type the 9 PM version. Save the surveyor version.

Every free-text field in the record — incident descriptions, corrective actions, PIP notes, drill notes — has a ✨ Polish button. It rewrites your shorthand into documentation a surveyor can read, and it will not invent the parts you didn’t write.

  • Your facts, never altered
  • ANE language preserved exactly
  • You review before anything saves
Incident · DescriptionExample
What you type at 9 PM

clt fell in bathrm no injry maria called 9pm f/u am

What the record keeps

Client fell in the bathroom. No injury was noted. Maria was notified by telephone at 9:00 PM. Follow-up scheduled for the following morning.

✨ Polish

Illustrative. Polish never saves on its own — the rewrite lands in the field for you to read, change, or undo.

04Survey day

Surveyors don’t make appointments.

An HHSC surveyor can walk into your agency on any weekday morning. When they do, your QAPI program is whatever you can produce — not whatever you meant to organize.

For most PAS agencies, “the program” is a binder assembled the week before the last survey, a spreadsheet only one person understands, and the memory of a coordinator who left in March.

“Based on record review, the agency failed to provide evidence of an ongoing quality assessment and performance improvement program.”

05The rule, mapped

The regulation is the spec.

PasQAPI wasn’t designed around generic “quality management” ideas. It implements Chapter 558, requirement by requirement. Scroll — each rule maps to the screen that satisfies it.

26 TAC §558.287(a)

QAPI program

An agency must maintain a QAPI Program that is implemented by a QAPI Committee — ongoing, focused on client outcomes that are measurable, with a written plan of implementation.

06 — The clock

The 24-hour rule doesn’t care that it’s 2 AM.

An abuse, neglect, or exploitation allegation starts a legal clock the moment your agency learns of it. §558.249 gives you 24 hours to report — to DFPS and HHSC both. Here is one night, as PasQAPI records it.

Deadlines you can’t lose track of, timestamps you don’t have to reconstruct, and a trail that reads the way a surveyor expects it to.

07The index

Everything they ask for, indexed.

Six modules, one record. Each row is a question surveyors actually ask — open it to see where the answer lives.

The board opens to ten indicators with current values, targets, and green–amber–red status. Every value dated, every threshold visible.

10The fine print

Boring where it should be boring.

Compliance software earns trust with architecture, not adjectives.

10.1Your agency’s data is walled off at the database.
Every table is protected by row-level security scoped to your agency membership — enforced in Postgres itself, not just in application code. Your record is invisible to every other agency, and theirs to you.
10.2Three roles, deliberately few.
Admins manage the team, coordinators keep the record, viewers read it. A surveyor-day login can see everything and change nothing.
10.3Timestamps that survive an audit.
Every compliance timestamp is pinned to your agency’s timezone — not the server’s, not the viewer’s browser. A 2:14 AM call reads as 2:14 AM, in the record and in the PDF, forever.
10.4The record is yours.
Training history imports from CSV in minutes, and the survey report exports as a PDF you keep. No lock-in by hostage-taking.
